چکیده

Removing anchored spirals from obstacles is an important step in terminating cardiac arrhythmia. Conventional anti-tachycardia pacing (ATP) has this ability, but only under very restrictive conditions. In a generic model of excitable media, we demonstrate that for unpinning spiral waves from obstacles this profound limitation of ATP can be overcome by far field pacing (FFP). More specifically, an argument is presented for why FFP includes and thus can only extend the capabilities of ATP in the configurations considered. By numerical simulations, we show that in the model there exists a parameter region in which unpinning is possible by FFP but not by ATP. The relevance of this result regarding clinical applications is discussed. 4 Author to whom any correspondence should be addressed. Spatiotemporal dynamics of pacing in models of anatomical reentry

Reentry around non-conducting ventricular scar tissue, which often causes lethal arrhythmias, is typically treated by rapid stimulation from an implantable defibrillator. However, the mechanisms of termination (success and failure) are poorly understood. To elucidate such mechanisms, we studied pacing of anatomical reentry in 1-D and 2-D excitable cardiac media models. Our results suggest that ...

Suppressing arrhythmias in cardiac models using overdrive pacing and calcium channel blockers.

Recent findings indicate that ventricular fibrillation might arise from spiral wave chaos. Our objective in this computational study was to investigate wave interactions in excitable media and to explore the feasibility of using overdrive pacing to suppress spiral wave chaos. This work is based on the finding that in excitable media, propagating waves with the highest excitation frequency event...

Comparison of resetting and entrainment of uniform sustained ventricular tachycardia. Further insights into the characteristics of the excitable gap.

BACKGROUND Resetting and entrainment have both been used to characterize the electrophysiological properties of the reentrant circuit in ventricular tachycardia. Several entrainment studies have suggested that the circuit has decremental properties, because the return cycle increases at faster pacing rates. Resetting, however, demonstrates a fully excitable gap in the majority of tachycardias. ...

Electrogram analysis and pacing are complimentary for recognition of abnormal conduction and far-field potentials during substrate mapping of infarct-related ventricular tachycardia.

BACKGROUND Mapping to identify scar-related ventricular tachycardia re-entry circuits during sinus rhythm focuses on sites with abnormal electrograms or pace-mapping findings of QRS morphology and long stimulus to QRS intervals. We hypothesized that (1) these methods do not necessarily identify the same sites and (2) some electrograms are far-field potentials that can be recognized by pacing. ...

Controlling turbulence in excitable media by applying boundary periodic pacing and gradient force

– Spiral wave and turbulence control in excitable media by applying periodic pacing on system’s boundary is investigated. It is found that spiral waves can be always successfully controlled by boundary periodic forcing with suitable forcing amplitude and frequency. Turbulence caused by spiral-wave breakup is uncontrollable by the boundary pacing, whatever the forcing amplitude and frequency, if...
